You are correct on the 40 amp fuse.  It plugs into the underhood fuse center 
which is on the driver's side fender well, at least it is on my '01 2500hd 
with 8.1 gas engine and Allison auto transmission.  My brake controller 
instructions, Hayes-Lemmerz Energize III, also instructed me to put an 
inline fuse on the hot circuit before that connects to the controller but I 
chose to use an automatic reset circuit breaker 15 amp instead.  I have been 
told this was not a good idea because it might keep resetting until there 
was a fire.  I wonder what is worse, no brakes when you need them or getting 
information that something is not right, pulling off to the side of the road 
and pulling a wire off if the 40 amp circuit fuse has not blown yet.  I've 
had the setup for over 3 years and no problems.
