Harvey is right on.  The EZ-Lift is an old reliable brand with a good track
record.  My personal preference would be a Reese if I were to buy the whole
unit.

Both Reese and EZ-Lift have been around a long time, so you should be able
to pick up a used head at a modest cost.  However, a new head is not that
expensive, and compatible ones are also made by other good hitch companies
such as Valley Tow Rite.

My choice would be to go with a welded head and tow bar (you also will need
a 2" receiver for your tow vehicle).  However, if you are planning on
getting a new tow vehicle within the next year or two, then I would suggest
getting a bolt together adjustable head assembly. That way it would be a lot
easier to tranfer it to another tow vehicle (receivers are never the same
height off the ground on different vehicles).

Regardless, you will still want a friction sway control, which is an add on
in any case.  The advantage of going with a Reese hitch would be the ease
with which you can add their dual cam sway control system (as opposed to the
friction type).

Many folks even use the friction sway control with the Reese cam system.

Even with a heavy tow vehicle, if the trailer is over a couple of thousand
pounds I would go with the weight equalizing hitch.  It will keep the tow
vehicle level and maintain a good weight distribution on the front and rear
wheels, making for better handling and traction.

Remember, the brakes, tires, shocks, wheels and hitch are the only things
between your behind and the road.  So this is not a place to go cheap
(frugal yes, but skimp no).
