Don Hardman      For years I have carried in my purse, in my A/S right 
inside the door,   in my tow vehicle and in my VW  and  on the inside of the 
window by the back door  two sheets....one with all
the medications I take and the name and phone number of the doctor 
prescribing it  and on the other sheet  is a list of  doctors, addresses 
& phone numbers....my SS number and Medicare#    and
suplemental insurnce info...name and address and phone number of the 2 
people who have joint Power of Attorney for Health Care....a statement 
that I am total laryngectomee and use an Electro Larynx
for all speech.....that I have Sky Med to emergency transportation and 
their phone number ...My childrens names, addresses and phone numbers 
including their work numbers....all because my Grand
Daughter insisted  when she was going for MS  in  Excercise Phisiology for 
Cardiac Rehab.    Too many people arrive at a hospital or are involved in 
an accident without health info.       Pearl in
