[A/S] In case of emergency form

I added a document to the files sections entitled "In case of emergency"
that I have been working on. The purpose of the document is to have
something in my tow vehicle while we are traveling that will provide contact
name and phone numbers in case of an emergency as well as instructions for
caring for our little dog. I added a section to indicate where the trailer
is parked in case we were in an accident while the trailer was in an RV
park. This form is a culmination of other type forms that I have seen and I
hope it might be useful to someone else.
