As Mr. Hanson pointed out, the 19.5" ball height SHOULD make the 
trailer level, if the trailer springs haven't sagged.

You park the truck on level ground, put the hitch head in the 
receiver, and measure to the top of the ball.

What you have to consider is how much the ball is going to be lower 
when the equalizer bars have been set to put some weight on the front 
wheels of the truck and take it off the rear. (i.e. how high would 
the top of the ball be with the same weight as the tongue weight 
loaded in the front of the bed?)  If the tongue weight after 
equalizer adjustment is going to drop the ball an inch, you'd want it 
at 20.5" with no load.  If it's going to drop it 1.5", you'd want it 
21" with no load.  And so on.
