Some say after the break-in period, it quiets down a bit.  I think I 
noticed a bit of noise reduction around 20,000 miles.  I just 
recently installed 3 layers of b-quiet (www.b-quiet.com) in a 
friends '93 Cummins - a world of difference.  We had to tear the 
seats and carpeting out from the firewall to the back window.  That's 
not something I'm willing to do on my truck yet :)

Not sure which fumes you mean.  I don't get any exhaust fumes in the 
cab, but I do smell the breather bottle on the front of the block 
after I change the oil for about 500 miles.  Pop the hood and look 
between the fan and the block on the drivers side.  You'll need to 
empty that bottle every time you change the oil.  It *will* puke oil 
over your block and then be evenly distributed around the engine bay 
no matter what you do.  I put half of a paper towel in the bottom of 
the bottle to minimize the puking effect.  Others have relocated the 
bottle with a little plumbing.  After you drive on the oil for 
awhile, you won't smell the fumes anymore.
