It is a loose nut on a bolt. For  reasons I've never asked, Airstream
has, at least on the current style two hinge doors, used nuts and bolts.
No retainer plates, nut plates or whatever. So, on those rare occasions
when a hinge nut/bolt works loose or someone tries to take the hinge off
that way there is only one way to fix it. It is necessary to cut the
interior skin inside of the hinge to access and tighten the nut. On the
pre 94 coaches it is common practice to take a small piece of the inside
skin from under the galley to cover the hole. With the newer coaches the
inside fuzzy wall stuff can be glued over the hole.
