I would say that fatigue is the root cause of the failure.  I would suggest 
going to a place that specializes in selling fasteners and get a grade 8 
machine screw with the proper nut.  You should be able to find such a specialty 
fastener store in the yellow pages. This will give you a good, high strength 
fastener to work with.  If the stainless machine screw was purchased at a "regular" 
store, you may have been sold an imported fastener made of inferior material.
