BROADCAST NETWORKS FOR YOUR RV OR COMMERCIAL TRUCK If you are the owner of a 
Recreational Vehicle (RV) or a commercial truck driver you can receive 
Broadcast Network Programming including ABC CBS NBC and FOX. If you qualify you can 
receive up to two network packages from your choice of 14 cities!In order to 
receive these packages you must meet certain criteria set forth by law and 
submit a signed Declaration Form and other necessary documents to DISH Network. You 
can obtain a copy of the Declaration Form and instructions by downloading it 
here or by calling 1-800-333-DISH. The Declaration Form outlines the 
requirements you must meet in order to qualify for network programming.In addition to 
the completed and signed Declaration Form you will need to submit the following 
documents:RV Owners (1 Item): 
A copy of your current state vehicle registration 
Commercial Truck Drivers (2 Items): 
A copy of your current state vehicle registration 
A copy of your valid commercial driver’s license (CDL) 
Please allow 3 to 4 weeks to process your request.DISH Network is pleased to
