I had a similar problem and couldn't seem to solve it.  A shop guy simply 
placed a screw driver under the hinge, on the outside of course,  and 
levered it tight, which  in effect held the interior nut tight agains the 
panel, and then tightened up the screw.  So simple I blushed.

I had later had a second problem when I had to replace a cracked hinge on 
my '78.  I had to get to the inside to ge new nuts on.  Original nut fell 
down inside when I removed the screw holding the hinger thus had to get a 
new nut on.    I took a  1" hole cutter, and an electric drill,  and cut 
a hole in the interior wall.  I found the the original nut was held to 
panel with glue, looked like Vulkem chaulk, but nut had broken free.  
Well, anyway I was able to get new nut on the new bolt hinge and tighten 
up with socket.  Now I had a hole to cover.  At the hardware found some 
niftly snap in vinyl hole covers and  covered  the hole.  Did a neat job 
and is virtuallly unnoticeable .  You can tough up paint to match wall 
covering if you wish now .  At least if anything loosens in the future I 
can pop out the hole cover and reach the nut..  Incidently the hole I cut 
was right next to the door frame and thus had a little angle to reach nut 
with socket but it worked out. OK.
