Try checking the Yellow Pages for your nearest larger city or 
metropolitan area for "Matress" or Mattress "Manufacturers".  Most 
places where I have resided during the past fifteen or twenty years 
have had at least one specialty mattress manufacturer.  When my 
Overlander needed new mattresses, I went with a regional custom 
mattress manufacturer, Lebeda Mattress Factory ( 
http://www.lebeda.com/ ), who has several location across the upper-
midwest.  

You will find similar companies in most larger cities -- they will 
typically have a showroom where you can try out the various mattresses 
before placing your order.  From what I have been able to learn, most 
will have a slight up-charge for custom sizing -- Lebeda had a $75.00 
up-charge over the nearest standard size in the model line chosen.  
Some will work strictly from your measurements, while some will ask 
for a paper or cardboard template (particularly if a radiused corner 
is needed).
