It sounds like you may have excessive end play in the motor. This can 
cause the shaft to  move outward during startup, then as the fan  
settles in it will  work it's way back to where it belongs. If you can 
get to it you can check to see if  the shaft has excessive play in it by 
pushing or pulling the fan blades back and forth. Make sure that you 
have the power turned off to the fan, you don't want it biting fingers.
