Come up with a cost for milage, so much per (loaded and unloaded). As in 
how much Gas and such. and then prorate that against how many miles 
traveled.

I would consider this part of the fee (as a customer), and not want to 
go into charging seperately for it, but you may want to place a 
surcharge (multiplier) of some sort for the difference in
elevation.

And if your really trying to make it pay, look for the load on the other 
end to bring back   :-)
