Is this a one-handle control, or does it have individual faucets? Does
it have a spout to fill the tub or only a connection for the hose to the
shower?

My '68 had a 2-handle control, with no spout, but I don't know if it was
stock. I found a replacement that was "close" at a local supply house
and was able to adapt the fittings and bolt it in.
